Handle vanishing PIDs (#1043)

Browse source 
PIDs can vanish (exit) from /proc/ between gathering the list of PIDs
and getting all of their stats.

* Ignore file not found errors.
* Explicitly count the PIDs we find.
* Cleanup some error style issues.
