tsdb/index: use ScratchBuilder to create Labels

This necessitates a change to the `tsdb.IndexReader` interface:
`index.Reader` is used from multiple goroutines concurrently, so we
can't have state in it.

We do retain a `ScratchBuilder` in `blockBaseSeriesSet` which is
iterator-like.
