Bjoern Rabenstein 5859b74f1b Clean up license issues.
- Move CONTRIBUTORS.md to the more common AUTHORS.
- Added the required NOTICE file.
- Changed "Prometheus Team" to "The Prometheus Authors".
- Reverted the erroneous changes to the Apache License.
